    If you'd like Blessed Mama Creations to make a photo collage card or other photo collage gift for you from a photograph taken by a professional photographer, you must first get permission from that photographer. You can do this by printing the form below and taking it to the photographer. We refuse to make products from copyrighted photos without the consent of the photographer.

The sitting number (if applicable): __________________. This is for the customer's use and it will not to be sold. I also grant permission to Blessed Mama Creations to showcase my photos (in a photo card design or photo card gift) as a sample on their site, http://www.blessedmamacreations.com.
